D. What else helps to identify this person (hobbies, special foods, activities, etc.):
E. Personal Circumstances
Last Date Seen (or heard from): Last Time (of day) Seen:
What Were They Doing (going to school, leaving work, at a friend’s house, etc.):
F. Means of Transportation
What form of transportation did they have available to them (car, bicycle, train, bus, etc):
What form of transportation did they have, or were they on, that day?
G. Possible Reason Missing (even if not known to be true and... there might not be one):
This is about what you think may be the reason. Even if you are wrong, it gives the lead investigator some direction in the case. Even fragmentary information is helpful. Let the investigator sort it out; they are supposed to know what to do and how to do it. Information such as “recent custody dispute”, “argument with boyfriend last night”, “threats from a former employee”, and “I really have no idea” are all relevant until disproved by investigation.
H. Possible Person Involved:
If you suspect someone in particular, let the investigator know right away. Create an information sheet for that person the same way you have done for the missing person, with whatever information you have or can gather. Let the investigator determine if that person is involved or not... not you.
